- Ideal Situation(Standard) ( Optional )
- Problem Definition ( Mandatory )
- Current Situation ( Mandatory )
- Attachments ( Optional )
Instructions
- State the ultimate goal (purpose of the work) – ex: “We have no quality issues”
- State the standard/to be situation
- State the current situation as represented by facts
If you can show the trend over past days/weeks/month that is best
- Visualise (graph) the current situation vs standard and numerically show the gap
- If necessary, put in a temporary measure if there is a safety/quality/etc. risk.
